Preparing for your Initial visit:

For your convenience and to minimize your waiting time, we recommend that you complete some of our paperwork prior to your visit. Please download the appropriate forms below and bring the completed forms to your visit, along with any relevant past medical records. We also ask that you bring a photo ID and your health insurance card to each visit. 

If you are considering transferring to our practice during your pregnancy, please download and fill out the appropriate forms below and have a complete copy of your prenatal records, lab work and sonograms with you at your first visit.

Insurance/Billing information

We participate with most insurance plans, including Aetna, Alliance, Carefirst BlueCross, Cigna, MDIPA, Optimum Choice,and United Healthcare. For a complete list of  accepted insurance carriers, details regarding health insurance or self-pay options, please call our billing office at 301-414-2305.
